How referrals work

A senior nurse will review the clinical information and contact the patient and family to arrange the next step. Within seventy-two hours of the first clinical conversation, you will receive a written Action Memo to your nominated email address, with the client's consent.

What we can accept

Referrals for in-home nursing care, post-hospital transition support, structured in-home assessments, and telehealth-led Tier 1 Advisory anywhere in regional Queensland and Northern New South Wales.

Our full scope of practice is held by Laura Zimmerman, endorsed Nurse Practitioner. For services outside our scope we will let you know promptly and, where possible, suggest an appropriate alternative.

What we ask you to include

Patient name and date of birth. Primary contact, either family member or patient. Reason for referral. Relevant clinical history. Current medications. Any safety or urgency concerns. Your preferred method of reply.

Attach a current GP summary or discharge summary as a PDF where available.

You can attach summaries and clinical notes directly inside the referral form.
